What support do you need as a carer?

What medication(s) is your family member taking at the moment?

Are they reluctant to take this medication?

What effect has the medication had?

Have you got a list of phone numbers to contact the crisis and home treatment teams out of hours?

Has your family member signed an advanced directive so that we could discuss the details of their case if they are not able to, or in crisis?

Do you think the person you care for could manage the interaction with other members of a group?

Do you think they could accept objective criticisms?

Have you tried contacting voluntary organisations active in your area that run support groups?

Do you have a list of the relevant agencies that deal with financial matters like Carers Allowance, Disability Living Allowance and prescription exemption?

Have you been offered a visit from a Carers Assessment Officer who will evaluate YOUR needs and make the necessary recommendations?
